MIAMI, FL (April 7, 2025)  The Health Foundation of South Florida is proud to announce that President and CEO Loreen Chant was a featured speaker at the 2025 RISE Summit on Social Determinants of Health (SDoH), a premier national convening focused on addressing the root causes of health disparities.

Chant joined a powerful panel of changemakers, including Monica Figueroa King of Broward Healthy Start Coalition, Amy Pont of Community Care Plan, and Dr. Catherine Parker Toms of Green Cars for Kids. Together, they explored what it takes to build more equitable health systems, ones that are grounded in the lived experiences of individuals and communities and centered on solutions that go beyond the clinical setting.

Held annually, the RISE Summit brings together healthcare leaders, community-based innovators, and policy experts from across the country to share actionable strategies for advancing health equity. The event serves as a critical platform for generating bold ideas and scalable solutions that address the social drivers of health, such as housing, food security, transportation, and economic opportunity.

“At the Health Foundation, we believe that health equity is not just a goal—it’s a shared responsibility,” said Chant. “When we align our efforts across sectors and geographies, we accelerate our collective impact. That’s how we create a system that delivers better outcomes, lowers costs, and truly serves our most vulnerable neighbors.”

The Health Foundation of South Florida is honored to stand alongside national leaders driving this important work and remains committed to investing in solutions that meet people where they are—because everyone deserves the opportunity to live a healthy life, regardless of their ZIP code.
