Organization: Baptist Health Foundation
Mission: to improve the health and well-being of individuals and promote the sanctity and preservation of life, in the communities served guided by faith in Jesus Christ with a vision to be the pre-eminent healthcare provider, a national and international leader in healthcare innovation.
Project Title: SBIRT: Screening, Brief Intervention, and Referral to Treatment
Grant Amount: $60,000
Project Period: 1/15/2011-1/15/2012
Purpose: Adopt an evidence based substance abuse screening and follow-up process into the standard protocols of South Miami Hospital's Emergency Department. SBIRT is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with substance use disorders, as well as those who are at risk of developing these disorders. SBIRT is an approach that aims to screen individuals for substance use to identify and address substance abuse concerns through brief intervention and referral to treatment services to 100% of positive screens, with a goal to reduce prevalence of substance abuse.

Organization: Health Council of South Florida, Inc.
Mission: to be the source of unbiased health data; provide quality program planning, management, and evaluation; create strong community partnerships and advocate for targeted healthcare policy at the local, state, and national levels. The HCSF is dedicated to community planning that improves health and promotes wellness in Miami-Dade and Monroe Counties.
Project Title: Health Council’s Miami Matters
Grant Amount: $30,000 & $48,000
Grant Period: 1/1/2011-1/1/2012
Purpose: Help establish an on-going, Web-based, Miami Dade County specific health data repository; Miami Matters. Miami Matters provides a community snap shot for Miami-Dade County. Updating and sharing of data related to the health and quality of life outcomes desired by the community are critical to better analyses and planning, providing for a broader picture of our community and the strategies for change. The site is geared towards planners, policy/decision makers, program managers, grant writers, educators, students and individuals and seeks to provide the data required for informed decision making on projects, plans, policies and other actions. Through Miami Matters, The Health Council aims to advance evidence-based planning, policy development and resources allocation in Miami-Dade County.

Organization: Hospice by the Sea, Inc.
Mission: to provide skilled medical care and comfort to patients with life-limiting illnesses, and emotional, social, and spiritual support to patients as well as their families regardless of age, race, creed, diagnosis, or ability to pay.
Project Title: Electronic Medical Record Implementation
Grant Amount: $40,000
Purpose: Purchasing, installation and adoption of qualified EMR software to facilitate use from remote locations. The EMR will facilitate all field staff's ability to access software for electronic charting for documentation of patient information and care plan interventions. This will improve the quality, safety and efficacy of patient care throughout all of our delivery settings. The project will provide the infrastructure Hospice by the Sea needs to fully implement the Electronic Medical Record for all patients.
